Residents of all diagnoses and functional levels can thrive in this clean and tidy environment. Poplar Care offers a hair salon, a beautiful courtyard, and a thriving garden, giving it a feeling of home. Poplar Care also offers spousal accommodations, private suites for rehab-to-home patients, weekly church services, individually controlled thermostats, and a robust activity program with community involvement focused on music, crafts, gardening, games, and outings that will make any resident feel included. Pets are permitted to visit the facility as well! Poplar Care has a fantastic dietary team that is led by a top-notch chef that works in unison with a licensed dietitian, who provides routine visits to our residents. Poplar Care has a full-time licensed social worker on staff and Dr. Kiran Burla and Leanna Graber, NP provide excellent medical services for our in-house residents. The medical team offers audiology, podiatry, dentistry, optometry, and psychology services, as well as transportation to and from medical appointments in the facility-provided Care Strategies transportation van. Of all the great things Poplar Care has to offer, the gem of the facility is the therapy gym. Boasting one of the largest therapy gyms in Southern Indiana, stocked fully with equipment for all diagnoses, Poplar Care offers premium Rehab-to-Home services. With a Physical, Occupational, and Speech therapy team that has been in the facility for 20+ years, the outcomes we generate at Poplar Care are second to none. Therapy services at Poplar Care don’t stop after a skilled stay though! Poplar Care also offers excellent therapy services to our long-term care residents, as well as offering outpatient services to patients in the community, complete with a private outpatient therapy parking lot, entrance, and waiting room. Our facilities accept new residents with Medicare, Medicaid, Anthem, United, Humana and some VA benefits along with Private Pay.
